ИНФЕКЦИЯ, ВЫЗВАННАЯ ВИРУСОМ ГЕРПЕСА ЧЕЛОВЕКА 6 ТИПА, У ДЕТЕЙ ПОСЛЕ АЛЛОГЕННОЙ ТРАНСПЛАНТАЦИИ ГЕМОПОЭТИЧЕСКИХ СТВОЛОВЫХ КЛЕТОК В РЕСПУБЛИКЕ БЕЛАРУСЬ

Цель: оценить частоту реактивации ВГЧ-6 инфекции у пациентов после аллогенной трансплантации гемопоэтических стволовых клеток (ал- ло-ТГСК); определить вирусную нагрузку в сыворотке/плазме крови и связанные с ней клинические проявления; определить критерии начала противовирусной профилактики и этиотропной терапии у пациентов с данной инфекцией в Республике Беларусь.

Материал и методы: мониторинг ПЦР ВГЧ-6 в сыворотке/плазме крови у 42 пациентов после алло-ТГСК.

Результаты: у 31% пациентов на 14-28 сутки после алло-ТГСК выявлена ДНК ВГЧ-6 в сыворотке/плазме крови. Концентрация ДНК ВГЧ-6 в сыворотке крови достигала 2,3-6,5×103 копий/мл, у 3 пациентов (18,8%) с концентрацией ДНК≥2,3×103 копий/мл развивались клинические проявления в виде гепатита. Регулярный мониторинг ВГЧ-6 инфекции позволил выявить реактивацию инфекции и при вирусной нагрузке ≥100 копий/мл начать своевременное профилактическое лечение ганцикловиром.

Заключение: ДНК ВГЧ-6 выявляется у трети пациентов после алло-ТГСК, причём в виде гепатита. Своевременная профилактика и терапия ганцикловиром снижает риск тяжёлых осложнений и смертельных исходов.
