КОМПЛЕКСНОЕ ЛЕЧЕНИЕ МЕСТНОРАСПРОСТРАНЁННОГО И МЕТАСТАТИЧЕСКОГО РАКА ЖЕЛУДКА: РОЛЬ НЕОАДЬЮВАНТНОЙ ВНУТРИАРТЕРИАЛЬНОЙ ХИМИОТЕРАПИИ

Цель: проанализировать современные подходы к лечению местнораспространённого и метастатического РЖ с акцентом на место внутриартериальной химиотерапии (ВАХТ) и её возможное влияние на вероятность выполнения R0-резекции, патоморфологический ответ, включая частоту полного патоморфологического ответа (pCR).

Материал и методы: поиск литературы выполнялся в PubMed/MEDLINE, Scopus, Web of Science и РИНЦ (eLIBRARY.ru) за 2014–2025 гг. с использованием ключевых слов MeSH: «gastric cancer», «neoadjuvant chemotherapy», «regional chemotherapy», «intra-arterial chemotherapy», «hyperthermic intraperitoneal chemotherapy», «conversion therapy», «perioperative treatment», «hepatic metastasis» и русскоязычных аналогов. Идентифицировано 642 публикации; после устранения дубликатов и нерелевантных работ в анализ включены 48 статей. Приоритет отдавался РКИ, проспективным когортным исследованиям и метаанализам.

Результаты: использование ВАХТ в неоадъювантном режиме продемонстрировало увеличение частоты выраженного патоморфологического регресса и pCR по сравнению с изолированной системной ХТ. Во многих сериях отмечено повышение доли R0-резекций, включая случаи исходной нерезектабельности. При этом профиль системной токсичности, как правило, не превышал аналогичный при внутривенном введении цитостатиков, однако требовал контроля локальных сосудистых осложнений.

Заключение: ВАХТ представляет собой перспективный компонент мультимодального лечения, способный повысить шансы на радикальное хирургическое вмешательство и улучшить локорегионарный контроль заболевания. Дальнейшая оценка её эффективности требует проведе- ния стандартизованных проспективных исследований с унифицированными критериями отбора пациентов.
