ANALYTICAL ASSESSMENT OF HEALTHCARE SERVICE DELIVERY INDICATORS IN THE REPUBLIC OF TAJIKISTAN

Objective: To conduct a comprehensive analytical assessment of service delivery indicators (SDIs) in the primary health care (PHC) system of the Republic of Tajikistan based on an analysis of infrastructure, resources, health service delivery, and outcomes, to identify priority areas for improving the quality and accessibility of care

Methods: A national study of 490 PHC facilities was conducted using simple random sampling, including 36 urban health centers, 52 district health centers, 210 rural health centers, and 192 health houses. The study included 1,266 health care workers and 2,479 patients and was conducted from September 2024 to January 2025. Standardized assessment tools were used to evaluate the structure, processes, and outcomes of health care delivery.

Results: Laboratory services were available in 52.7% of facilities, while 99.5% had access to medicines. Only 10.1% of facilities had full emergency transportation capacity. The average physician density was 0.34 per 1,000 population. Limitations were identified in information and communication technologies (ICT), water supply, and electricity reliability. Management practices were generally well developed but require further strengthening at rural health facilities.

Conclusion: SDIs are an effective tool for assessing health system performance. The identified disparities require targeted managerial interventions to improve the accessibility, quality, and sustainability of PHC services.
