Revascularization in distal lesions of upper extremities arteries
Etiological factors of peripheral arterial occlusions of the upper extremities are: obliterated tromboangiit, post-traumatic, postthrombotic occlusion, Raynaud's disease and atherosclerosis. Upper extremities ischemia in obliteration of distal blood stream becomes critical.
Surgical revascularization in this situation is difficult. Direct revascularization is possible only in patency of distal vein, which is a small gravity. Palliative non-standard methods of revascularization (cervicalthoracic sympathectomy, venous arterialization, autotransplantation of major omentum, etc.) often avoid amputation.
The use of precision technique and microsurgical tools allow to perform reconstructive surgery on the arteries of small caliber, the use by the slightest chance of revascularization to save the limb.
Questions of choice operation method, depending on the disease etiology. the nature and severity of upper limb ischemia, remain poorly studied.
