Our experience of surgical treatment of juvenile angiofibroma located in the skull base

The authors propose a temporary external carotid artery ligation on the side of lesion before the removal of tumor in patients with juvenile angiofibroma of the skull base (JASB).Taking into account the proposed method, the patients were divided into two groups: I group (n=10) - without first temporary ligation of the external carotid artery (the traditional method) and II group (n=25) - with a preliminary temporary ligation of the external carotid artery.

The intraoperative blood loss in patients of group I was 600,0-1200,0 ml, which, in turn, made it difficult to visualized the surgical field and forced to blood and plasma transfusion. Recurrence of the disease in this group was noted in 1 patient.

Bleeding during surgery in patients of group II ranged from 250.0 to 550.0 ml, i.e. in 2.0 and 2.5 times less than in group I, that provide better visualization of surgical field and did not require a transfusion of blood components. In this group no relapse was observed.

Comparative analysis has shown the effectiveness of proposed surgical treatment of JASB, so it can be used as an alternative to embolization of the external carotid artery basin, in all regions of the republic, ie, in those place where no high-tech equipment is not existed.
